Peter*, 56, has Down syndrome and a rock-solid daily routine. When support workers noticed he was sleeping through breakfast but awaking the whole night, they logged every change.

The Service Manager helped Peter’s family obtain a functional assessment; results showed early dementia.

Partnering with the Support Coordinator, Upwards Living prepared evidence for a early plan review—securing extra hours for cognitive stimulation activities and evening supervision.
